You will be asked to fill this form in once you are approved for adoption. Please fill this form in and either hand it over to the carer, along with the change of owner form or email back to us.

Wanting to become a foster carer?
If you are wanting to become a foster carer, please fill out this form and email back to us, including your driving licence.

DESEX DECLARATION FORM
You must sign a declaration for for all dogs under 6 months old.
If you new have adopted a puppy under the age of 6 months you will need to fill out the Desex Declaration form and return it to us jadesdogrescuensw@gmail.com

Once you puppy has reached 6 months old (Some larger breeds 12 months old) You will need to contact us and we will book your desexing procedure in at one of our nominated vets.
CHANGE OF OWNER FORM
Change of owner form for council.
You will need to fill in a change of owner form when surrendering a dog to us OR receiving an adopted dog from us. The 'Change of Owner Details C3A' form will be sent in to local councisl. The form is also available on our website at www.dlg.nsw.gov.au. Three copies need to be signed, one for us, one for council and one for the new owner.

Tips on owning a retired greyhound.
Can I let my greyhound off-leash? As a member of the sighthound family and one of the fastest animals on earth, we recommend that if you do wish to let your greyhound off-leash, it is done so only in a completely enclosed area (i.e. fenced and designated leash-free area). We do recommend that even if your greyhound has its Greenhound muzzling exemption, that you keep his/her muzzle on due to the heightened level of activity and excitement which is quite common in dog parks. Jade’s Dog Rescue discourages taking your adopted greyhound to a Dog Park.
